What should I bring to my first appointment?
- Some insurance plans require a signed prescription from your physician including: diagnosis, frequency and duration of care. If you don't have a prescription please see FAQ "Do I need a prescription".
- Medicare patients must bring a detailed list of all medications, vitamins and any
herbal supplements with name, mg, and frequency of each item.
- Your current insurance card(s) and picture ID
- Co-pay, co-insurances and deductible payments due at the time of service (if applicable).
We accept Visa, Mastercard and cash payments only
Do I need a prescription?
- Some insurance plans require a prescription for physical therapy. If you plan to use your insurance and do not have a prescription (self-referring), we may be able to see you for 12 visits or up to 45 days without a prescription depending on your insurance plan.
- If you are self-referring and do not have insurance, the therapist can treat you and will discuss your plan of treatment and number of visits directly with you at the time of your evaluation.
What should the prescription look like?
- Some insurance plans require a signed prescription stating the patient's diagnosis, frequency, and duration, etc.
- You may print a full-size prescription form or the physician may utilize their own form.
- The prescription can be faxed to us at 858-866-0342, brought into the office by the patient or mailed to: Ocean Physical Therapy, 4501 Mission Bay Drive, Suite 3K, San Diego, CA 92109
How long are the appointments?
- Plan to come in 20 minutes before your first appointment time to complete your paperwork. The comprehensive evaluation will run about 45 to 60 minutes.
- Each additional visit will be 45 to 60 minutes and consist of exercises in our gym, manual therapy, and modalities as needed.
What should I wear?
- Please wear comfortable clothing or gym attire that allows for complete range of motion so that the therapist can fully access the area being treated. Close toe shoes are required in the gym.

- Parking is free. There is a parking lot in front of the building and also behind the building.
